Should any employee have an accident, it is critical that you immediately notify Companion Property & Casualty. This allows our professional nurse case managers and claims adjusters to ensure that your employee receives the most appropriate care and cost-effective medical treatment. Timely reporting also has a favorable influence on the premiums that you pay.

Immediate notification of claims allows us to properly investigate and evaluate the claim, as well as to effectively manage the medical care that your employee receives. As soon as an injury occurs, send us a "First Report of Injury" form. Please note that submission of the "First Report of Injury" form is not an admission of any type of liability. First Reports of Injury may be submitted as follows:

Submit via fax at: 1-877-379-7389 - using a copy of the "First Report of Injury" form you received with your policy, or by downloading off of our website by clicking on this link Workers Comp Forms, followed by the state selection for the state where the injury occurred.

However the claim is reported, you should be sure to do so in the most expedient manner and be sure to send the employee to one of the physicians on the Companion Property & Casualty physician panel.
