We understand how important information is to your business. Here is where you will find a variety of helpful reports.
Certificates of Insurance
- Assigned risk customers – Here you can get a workers' compensation certificate of insurance on a current policy. You will need a valid policy number.
- All other customers – You can get a certificate of coverage from your agent.
Loss Runs
- Assigned risk customers – Here you can find your own loss run report on a current policy.
- All other customers – You can get a loss run report from your agent, or by completing this request form.
Third Party Administration (TPA)
If you're self-insured through Companion TPA, we realize you have unique data reporting requirements. We provide you with recurring reports, customized for your business. We also provide a secure self-service tool for you to access your own historical data. For information on how to access this service, please contact your account manager.
