You will also find links to repositories of forms that are used by the system in your state, and access to important safety and health information provided by various state agencies.

The section labelled "Network Medical Providers" will connect you to a comprehensive directory that lists all medical providers in your area, who currently participate in Companion's medical network.

Lastly, and very importantly, we have provided a "Claim Service Survey" for you to give us feedback on your experience with our claim process. In the unfortunate event that you have suffered a workplace injury or illness, we would hope that our claim service has made the effort of recovery and rehabilitation easier.
